Club MSM Alpine Ski Members’ Code of Conduct and Responsibilities
Club MSM Consent, Assumption of Risk, Waiver and Indemnification

STEP THREE: Please complete STEPS ONE and TWO and email the completed forms to sarah@sjburroughs.com. Please note that both Athletes and Parents must sign the Code of Conduct, and only Appendix A of this document must be submitted with the other form.
